As a leading dental practice within the community we are committed to improving and maintaining excellent oral health for all our patients. We pay close attention and focus to our patients needs whether it pertains to treatment needed, or to customer service and billing issues. We feel it is imperative to work hard as a team to ensure all our patients feel as important as they are no matter if they are new or existing patients within our community so that they know we are truly here for them. As a gift to our new and existing patients we are excited to offer Free Whitening for Life Program. Patients who currently have a insurance plan are still eligible to participate in the program for a onetime fee of $59.

This offer, while absolutely free, does require some general rules, regulations and limitation. Please read through the policy carefully. While our Whitening for Life Program is a special gift from us to you, we stand firm on our rules and regulations and insist that they are followed in order to receive full benefits of this program.

Lifetime Maintenance Rules and Regulations

  1. Patients must maintain minimum continued care for their personal treatment plan from our hygienist. Most likely this will require patients to honor their hygiene schedule of at least two visits a year at approximately 6 month intervals. This regiment helps reduce dental expenditures and the need for any emergency care.
  2. We have the right to deny the offer based on patient current or future health conditions. We, individually and collectively hold the patients best interest as our first and foremost responsibility and without proper gum and teeth health, whitening could be damaging to your oral condition.
  3. Patients must comply with all policies regarding payment and missed appointments.
  4.  As a common courtesy our patients are required to keep all scheduled appointments (any cancelations must be made within a 48 hour minimum prior to scheduled appointment and should be rescheduled within two weeks).
  5. Patients are also required to keep payments up to date and not have any outstanding bills with our dental office- it must be paid within 30 days of treatment.
  6. If patients fail to meet the above requirements, we will provide a one time opportunity to re-enroll in the program for a $50 reactivation fee.

Our responsibility as your dentist

  1. The office will put together custom/professional trays for patients.
  2. Patients will schedule an appointment for custom impressions of their teeth. After impressions are made, whitening will be ready for pick up within 48 hours.
  3. The office will then provide our patients with their own set of whitening trays without cost. We will then provide patients with one complementary whitening gel syringe upon each hygiene appointment annually.
  4. Lost or destroyed trays can be replaced at a cost of $30 per tray to the patient. After 3 years we will replace both top and bottom trays due to wear and tear without charge.
  5. If patients fail to meet any of the rules, they will be disqualified from the program but will be provided a one time opportunity to re-enroll for a $50 reactivation fee.

Patient activation guidelines

  1. Must be at least 18 years or older to participate in the Whitening for Life Program.
  2. Must complete hygiene cleaning, x-rays, doctor’s exam and schedule patients’ next 6 month appointment.
  3. Patients are required to stay current with their recommended dental health appointments and maintenance.
  4. Must comply with the minimum required dental care as treatment planned by doctor. This treatment plan will vary upon patient.
  5. No active dental cavities or active periodontal disease.
  6. Once all necessary dental treatments have been completed, patients will then have impressions taken and trays will be delivered within 48 hours.
  7.  Patients must keep track of all scheduled appointments, and any cancellations must be made within 48 hours prior to appointment.
  8. To keep receiving our special offer, patients must keep account balance up to date and paid within a 30 day period.
